3. Complete your new-patient forms
New patients may be asked to complete registration, medical history, privacy and financial forms before the first appointment.

4. Transfer your child’s medical records
If your child has previously seen another pediatrician, please arrange for the previous office to send your child’s medical records to Step By Step Pediatrics at fax number 480-776-0444

Transferring From Another Pediatrician
Families change pediatricians for many reasons. Our team will help make the transition as smooth as possible.

Payment and Office Policies
Copays, deductibles, and other patient-responsibility amounts may be due at the time of service. Step By Step Pediatrics accepts cash and credit cards. Self-pay patients are welcome, and payment plans are available.
Please provide at least 24 hours’ notice when canceling or rescheduling an appointment. Late cancellations and missed appointments may be subject to a $25 fee per appointment.
Patients who arrive late may need to reschedule, depending on the provider’s availability and the remaining appointment time.
